Your First Visit
After being greeted by our friendly receptionist and choosing the beverage of your choice (water, tea, coffee, sparkling water), you’ll be escorted to your treatment room where an assistant will begin your appointment by taking a few diagnostic images and X-rays. You’ll then get to meet Dr. Shahrestany, who will spend a little time getting to know you before performing a comprehensive oral exam. Afterward, she’ll go over her observations with you and discuss any additional treatments you might need. From there, you’ll have the chance to ask as many questions as you like.
Patient Forms
We pride ourselves on running a very punctual office, so to help you save time starting at your very first appointment, you now have the option to fill out our patient forms before you come to see us. Click on the link below so you can download and complete the forms at your leisure. When the day of your visit arrives, just bring the forms with you, and we’ll get you in to see Dr. Shahrestany right away! If you have any issues accessing our forms, please let us know right away.
